EC Charter Bus Reservation Form

Please reserve your bus tickets by filling out below form. Reservations are accepted beginning June 16 (Tue). You will be contacted after the deadline to confirm your spot; please contact Niigata Kotsu if you do not hear anything by Aug. 7 (Fri). If you have trouble sending the form through this site, please download this Reservation Form (PDF) and fax or mail it Niigata Kotsu Sado and keep the original with you. If you send your form by mail, please make sure you keep a photocopy.
